7th Grade, 8th Grade, Sophomore, Junior, Senior Athletes Only

State of Illinois Full Physical Form
ALL Freshmen Students and 6th Grade Students are required to get a Full Physical whether they play sports or not. A Full Physical covers the sports requirement as well.
